Steps To Take If You Are A Victim of Identity Theft:
- File a police report: get a copy of the report or a case number. It's your proof of the crime when dealing with creditors holding the fraudulent accounts.
- If phishing or other online fraud, contact Guaranty Bank immediately at 414-362-4636 or
1-800-235-INFO (4636). - Call one of the three major credit bureaus:
- Have a "fraud alert" placed on your file so new credit cannot be given without your approval. Each credit bureau automatically alerts the other two.
- Request and review your credit report carefully. You are entitled to one free credit report from each bureau every 12 months.

- Notify the companies that hold fraudulent accounts in your name and close them. Use the Federal Trade Commission's ID-theft affidavit to simplify your dealings with companies holding the fraudulent accounts. (www.consumer.gov/idtheft/pdf/affidavit.pdf)
- Change all of your passwords. Add passwords to accounts that don't have them. Don't use passwords that can tie back to you such as your mother's maiden name or a pet's name.
